Templates and Forms
The following files are basic templates and forms that Monica and I seem to be asked for frequently. Feel free to download them and change them however you see fit.
- This book checkout form can be used to help keep track of books lent to students, and hold them responsible for its return. Have the students fill it out when they borrow a book. Sign the bottom and give it back as a receipt when they return it.
- At the end of the term, it is good to ask the students for feedback on the class. Feel free to download this student evaluation form and modify it as you see fit.
- At Parent-Teacher conferences, you should always keep a record of who has attended and how you can reach them. Print out this Parent-Teacher Conference Sign-In sheet to use.
- If you are in a science classroom, it is important that both parents and students have read and signed off on appropriate laboratory behavior. You should have a Lab Safety Contract on file for all students before they are allowed in lab.
- Because transportation issues and alternate lunches make it difficult to schedule detentions, some teachers use these sheets as penalty for inappropriate behavior. Keep the two notification sheets (Respect Sheet Notification, and Talking Sheet Notification) on hand and cut almost in half. When a student doesn't respond to a verbal warning, complete one sheet and quietly leave the left half on the student's desk. Keep the right half for yourself as a record and reminder that the student must complete a sheet for you. At the end of class, present the student with the appropriate form (Respect Lesson or Talking Lesson) to copy.
